Evaluation Questionnaire Terms & Conditions
1. Open to all nurseries and pre-school settings completing and returning an evaluation form. Not open to employees of Organix Brands Ltd and their families, its agents or anyone professionally connected with the promotion.
2. Evaluation Form must be returned by 31st December 2010.
3. Winners will be drawn by an independent adjudicator.
4. The promoter’s decision is final. No correspondence will be entered into.
5. Prize fund consists of a Taste for Life workshop with Lucy Thomas at a time to mutually agreed by Organix Brands Ltd, Lucy Thomas and the prize winner and 25 sets of Organix Goodies worth £20.
6. Winners will be notified by email within 28 days of the draw date. There are no cash alternatives available.
7. Only one entry per establishment may be made.
